Clinical and biochemical parameters associated with substance-induced psychotic disorder: which differences between alcohol, cannabis and psychostimulants

Introduction According to DSM V, substance-induced psychotic disorder is a mental health condition in which the onset of symptoms can be traced use psychotropic substance. The pathogenesis this disease still poorly understood; current literature traces its causes back genetic predisposition and early traumatic events (i.e. child abuse). Objectives present study aims identify specific clinical features biochemical markers could addressed as predictors for long-term prognosis disease. Moreover, we aim correlations between phenotype underlying substance abuse, order allow start tailored treatment. Methods Between 2020 2022 recruited 218 patients referring Policlinico Hospital Milan San Gerardo Monza, Italy. All were diagnosed with induced disorder: 31 reported alcohol abuse (14,2%), 71 psichostimulants (32,6%), 116 cannabis, (53,2%). For each patients, collected demographic data, medical records comprehensive psychometric assessment (GAF, PANSS, BPRS, Modified Sad Person Scale-MOAS). Furthermore, blood sample dosing Na+, K+, Na+/K+, hemogram formula platelets, glucose , urea, creatinine, uric acid, transaminases, γGT, bilirubin, plasma proteins, albumin, LDH, CPK, PCHE, cholesterol, HDL, LDL, Tg, TSH. Results Chi squared test (χ²) has been used compare qualitative variables 3 subgrous (alcohol-, psychostimulants- cannabis-induced syndromes) (fig.1). One way ANOVA quantitative same subgroups (fig.2). After removing one subgropus (alcohol-induced symptoms), analysis have repeated. Significant included binary logistic regression model confirm their validity cannabis- psychostimulants-induced disorders (fig 3). Finally performed Omnibus Hosmer-Lemeshow verify these models. Alcohol syndrome: higher age hospital admission, cholesterol hurea levels, high comorbidity conditions anxiety/depression, low social functioning, suicidal risk;, hospitalization rate.
